Job Description:

Overview:

The Case Manager oversees file and case management for all enrolled and assigned youth, and acts as the centralized information holder for the youth. The Case Manager will work with individual participants to conduct in depth case management needs assessments that identifies the needs for each participant and to also remove barriers for the client’s employment, health and overall well-being.  The Case Manager will identify resources and services to address those needs.  The Case Manager supports the participant’s access to services by developing a personalized case plan by working with the participant to meet the goals of the case plan. The Case Manager is also responsible for determining youth/program eligibility, maintaining documentation requirements, and data input. Case Management may require intensive case management which includes: outreach, workshop facilitation, home visits, re-engagement, among other tasks.  Case manager will track progress of youth from the program period to the last quarter of follow-up.

 

Responsibilities:

  • Oversee implementation of services for each participant.
  • Complete case plans and other documentation following program policies and protocols.
  • Provide and maintain appropriate data and files of clients, including physical participant file and digital database.
  • Meet individual and programmatic benchmarks and outputs.
  • Provide transportation vouchers and other supportive services. Collect and verify all timesheets. Monitors program completion of each participant and adjust to meet program and participant goals.
  • Attend/plan/facilitate other department/agency/special initiatives staff meetings, trainings, professional development, or workshops, as assigned.
  • Assist in development and coordination of special assignments and programs.
  • Meet participant and/or families in a variety of settings including but not limited to: schools, worksites, home, court, other community based organizations.
  • Ensure that youth are prepared for, and connected to, the appropriate educational or employment opportunities.  Assist with placement in education track or employment. Helps create a resource library of information on education and training programs, internships, and other post-program opportunities.
  • Creates action/improvement plans for participants when necessary and ensures its follow-through. Enforces intervention services when necessary.
  • Report all data into required data management as prescribed by the grant and department in a timely manner.
  • Follow performance management systems.
  • Complete required reports and update appropriate stakeholders (Senior Case Manager, partners, other managers) in a weekly, bi-weekly, and/or monthly basis, as needed/requested.
  • Coordinates with other staff members to provide community and financial resources to participants.
  • Follow staff program manual.
  • Attend community events, forums, and other events that relate to the goals and objects of the program.
  • Travel as needed for YPI business including training, meetings, home visits, events, among other.
  • Maintain a professional attitude, confidentiality, and respect cultural differences.
  • Work collaboratively with other staff.
  • Handle crisis and/or emergency situations as needed.
  • Assist with Center duties, which may include but is not limited to front desk coverage.
  • Complete all other duties as needed.

Job Requirements:

Qualifications:

  • Bachelor’s degree in social work or related field strongly preferred.
  • Three (3) years’ experience providing case management services.
  • Two (2) years working in a non-profit organization environment.
  • Experience working with youth from at-risk environments, ages 16-24.
  • Excellent oral and written communication skills; ability to make group presentations and facilitate group activities.
  • Ability to work in a fast-paced, dynamic environment.
  • Ability to work effectively with minimal supervision, prioritize multiple tasks, manage competing priorities, meet deadlines with attention to detail.
  • Superior computer and organizational skills with significant experience in Microsoft Office and Google Drive.
  • Bilingual; English/Spanish preferred.
  • Able to work evenings and some weekends.
  • Must pass a criminal background check (Livescan fingerprinting), with both the Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI), prior to beginning employment.
  • Must present proof of negative Tuberculosis skin test (Mantoux) prior to beginning employment dated within 60 days.
  • Must have access to reliable transportation.  If applicable, will be required to provide proof of current California driver license, acceptable driving record (current DMV H6 printout), and current liability auto insurance.
